We’re Hiring : Patient Care Coordinator (The Ultimate People Person!)

Location : Houston, TX | Full-Time | $18–$23 / hr

Love helping people and keeping things organized? You might be our next superstar! At Harmony at Home Senior Care, we’re looking for a fun, fast-moving, compassionate go-getter to coordinate care, support our caregivers, and make sure our clients always feel like family.

Your Daily Adventures :

Communicate directly with clients and caregivers to ensure quality care and satisfaction.

Tackle scheduling changes, caregiver call-offs, and daily adjustments like a champ.

Help interview, onboard, and support new caregivers with a smile.

Assign training, track progress, and keep compliance documents in order.

Keep our databases (SwyftOps, GHL) clean, organized, and up to date.

Conduct check-ins, satisfaction calls, and performance reviews.

Pitch in with marketing events, referral partner outreach, and community engagement.

Provide administrative support and step in for the Administrator when needed.

What Makes You a Great Fit :

Previous experience in home care, home health, hospice, or similar healthcare setting.

Energetic, upbeat personality with strong communication skills.

Incredibly organized and loves solving problems.

Available full-time, in office (Monday–Friday, business hours standard).
