Location : Florence, KY

Schedule : Monday-Friday 8am-5pm, with occasional on-call and weeknight afterhours call

About the role :

The Palliative Care Nurse Practitioner exemplifies and promotes advanced nursing practice and excellence in Palliative Care, and provides comprehensive, quality care to individuals living with serious illness. A Nurse Practitioner in this position will focus on providing relief from the symptoms and stresses of a serious illness with the goal of improving the quality of life for patients, caregivers, and families. The Palliative Care Nurse Practitioner will be responsible for providing visits for both new and established patients, which may include visits to patients' homes, facilities, and hospitals. This position works in close collaboration with both community and Bluegrass Care Navigators' physicians, as well as our community partners through established palliative symptom management protocols.

The Nurse Practitioner plays an integral role in formulating a plan of care for the patient, family, and / or caregiver and assists with facilitating clear communication of this plan to Bluegrass Care Navigators' physicians, the patient's primary care provider, or specialist as appropriate. The Palliative Care Nurse Practitioner may help develop practice protocols for palliative and end-of-life care or educate other clinicians about the palliative care specialty while maintaining one's own professional knowledge and skills as an expert and role model in Bluegrass Care Navigators' advanced nursing practice.
