We have an exciting opportunity for a nurse to make a HUGE impact in a child's life. We are looking for a nurse that is ready to take a step back from the chaotic hospital environment and work one-on-one with a child in their home. Home Health nurses are heroes in Centria's mind. They afford these children and their families a piece of normalcy in their lives. Without home health nurses, these children would live at the hospital never knowing what it feels like to be home. YOU can make a difference- even if it is only one day a week.

Responsibilities

• Perform duties outlined in patient's plan of care
• TBI + SCI rehabilitation
• Pediatrics
• Geriatrics
• Ventilator and Tracheotomy care (Training available)
• Gastrostomy tube care
• Dispensing medications
• Observe client's physical, mental and emotional conditions.
• Report any changes
• Document observed changes
• Communicates effectively with Clinical team to increase optimal outcomes
• Maintains professional nature within the home setting
