POSITION TITLE : Case Manager

DEPARTMENT / PROGRAM : Portage Area Recovery Center (PARC)

REPORTS TO : Portage Area Recovery Center Program Manager

SCHEDULE : (Determined on a case by case basis)

GENERAL STATEMENT OF DUTIES : Providing substance abuse related case management to clients in PARC. Responsible for overseeing residents maintaining a therapeutic drug free environment following rules and regulations of F&CS and Ohio MHAS and COA standards crisis intervention and consultation / reporting with / to supervisor(s).

ESSENTIAL RESPONSIBILITIES :

sober living environment.

crisis intervention / de-escalation and when needed consulting with supervisor.

trusting relationship with clients while modeling awareness and behavior needed for growth and development of clients; displaying knowledge of the therapeutic importance of maintaining appropriate boundaries.

clients use of services / treatment and impact on independence.

individual and group treatment to clients.

drug screens as assigned.

client records in accordance with the State of Ohio regulations and F&CS policy / procedure.

in team planning case reviews and program goal achievement.

agency and training meetings as requested.

other duties as assigned.

with agency policies and procedures COA regulations federal and state requirements and educational / certification / registry requirements.

Requirements

QUALIFICATIONS :

Ohio Chemical Dependency Counselor Assistant licensure.

providing supportive services (monitoring case management crisis intervention AOD services etc.) for individuals in recovery.

to model open communication fair and equitable treatment and management of conflicts.

to work as a team member with people from diverse backgrounds and positions.

to work in stressful / crisis oriented environment.

Ohio Drivers License and ability to meet agency requirements for driving insurability.

be able to meet agency requirements for random drug screens criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).

communication and de-escalation skills.

to maintain appropriate therapeutic boundaries.

of and ability to display sober living and recovery skills empowerment of clients and the present a caring compassionate attitude.

MINIMUM EDUCATIONAL / LICENSURE REQUIREMENTS : Valid Ohio Chemical Dependency Counselor Assistance licensure

MINIMUM EXPERIENCE REQUIREMENTS : Minimum 2 years experience preferred.

PHYSICAL REQUIREMENTS : Must be able to take on-call. Ability to lift up to 25 lbs.
