Case Manager - Behavioral Health (Social Worker)

Job Title : Care Manager / Case Manager

Location : Remote (15-20% Travel Wayne and Macomb Counties, Michigan)

Duration : 3-4 Months Contract

We're looking for a compassionate Field Care Manager to support members facing behavioral health challenges. This role combines clinical expertise with community-based care, helping individuals access the support, services, and stability they need. This is a remote position with 15–20% local travel within Wayne and Macomb Counties.

Key Responsibilities :

• Manage a behavioral health caseload and conduct in-home, community, or phone assessments.
• Develop individualized care plans addressing physical, social, and behavioral needs.
• Collaborate with interdisciplinary care teams, physicians, and caregivers.
• Help members overcome barriers such as housing, transportation, and benefits access.
• Serve as the primary point of contact and advocate for each member.
