• Must have active state license
• BC required
• Weekdays, Days, 12-hour shifts
• Need 3 days per week (can be flexible)
• 20 - 22 patients per day, 15 - 20% peds
• Must be board certified in FM
• Credentialing needed
• DEA needed
• Paid malpractice insurance; pre-paid travel and housing expenses
• Assignment details and time entry in online portal
