Your Role as a PT

Deliver personalized in-home therapy sessions tailored to each patients needs and goals.

Develop treatment plans that improve strength balance and mobility reducing fall risks and promoting independence.

Educate patients and families on safe movement techniques and daily living activities.

Document patient progress and care using WellSky / Kinnser EMR.

Collaborate closely with physicians case managers and other care providers to ensure seamless care.

What Were Looking For

Licensed Physical Therapist in California (required).

AHA CPR Certification.

Minimum 1 year of experience. Home Health experience is a plus but not required.

Reliable transportation valid drivers license and auto insurance.

Strong communication organization and problem-solving skills.

Compassionate personable and self-motivated.
