The CNA is responsible for providing quality care to customers and supporting the goals for shift objectives to enhance team efficiency and customer satisfaction.Essential Duties and Responsibilities1. Provide and assist with physical care of patients which will include but not limited to bathing, oral hygiene, grooming, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation and range of motion2. Responsible for real-time documentation of patient care and observations3. Communicate timely with Licensed Nurse any observed or perceived changes in patient's condition or behavior4. Assist in the dining process by preparing customer for meals; serves and collects trays, assists in setting up and feeding residents, serves refreshments. Completes meal assessment and records oral intakes accurately5. Responsible for patient transport to scheduled therapy appointments6. Assists in recreational, motivational and socialization activities of care program7. Responsible for timely response to call lights and providing appropriate response or service8. Responsible for regular rounding with customers to anticipate and identify needs and respond timely and appropriately9. Fully understand all aspects of patient's rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting incidents of evidence of patient abuse or violation of patient's rights10. Maintain the comfort, privacy, and dignity of each patient in the delivery of services to them. Encourages growth and independence as care progression warrants11. Assists in maintaining the cleanliness and safety of customer room. Assists in maintaining a safe customer environment12. Adhere to infection control procedures including but not limited to hand washing and isolation practices13. Adhere to Par Scanning procedures to ensure accurate tally of supple usage and availability14. Ensure uniform and personal appearance is clean and professional15. Maintain clinical competence through completion of annual continuing education requirements16. May participate in interdisciplinary customer and service improvement initiativesCompetencies

• Thoroughly completes assignments

• Treatments and procedures are performed accurately and with proficient technique
• Demonstrates a proactive approach to complete assigned floor dutiesRequired Education / Experience
• High School Diploma or EquivalentRequired License / Certification
• Successful completion of a state approved Nursing Assistant Certification course
• Valid state CNA license
• Current BLS (CPR) certificationPreferred Education / Experience
