Position Title : Nurse Practitioner.

Reports To : Medical Director.

Job Summary : The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes.

Key Responsibilities :

• Patient Assessment : Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results.
• Diagnosis and Treatment : Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
• Preventive Care : Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
• Health Education : Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
• Care Coordination : Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.
• Telehealth : Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.
• Record Keeping : Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.
• Prescription Management : Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.
• Compliance : Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.
• Community Health : Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.
• Follow-ups : Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.
