Pinnacle Home Care Floridas largest Medicare-certified home health provider has been delivering high-quality patient-centered care for over two decades and were looking for Physical Therapists to join our award-winning team.

Key Responsibilities

• Perform initial and ongoing patient assessments including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families / caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patients condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services case conferences and meetings to promote coordinated care with healthcare team members.
• Identify patient and family / caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.
