What we're looking for in a Case Manager - NN Healthy Families
To excel in the role of Case Manager for NN Healthy Families, candidates should possess a Bachelor's degree in human services (such as Social Work, Psychology, Sociology, or Counseling) and at least one year of experience in case management services for children and adolescents. A valid Driver's License is essential for client visits and outreach activities. Strong communication skills, both written and verbal, are crucial for effectively collaborating with families, service providers, and other team members. The ability to conduct assessments, develop treatment plans, and provide counseling with empathy and professionalism is key to supporting clients in managing their physical, social, and personal needs.
