The Senior Patient Care Coordinator ( Optum San Bernadino Primary Care ) is responsible for providing assistance to patients in a friendly and courteous manner. The Senior Patient Care Coordinator is responsible for check-in and check-out functions, including registration and cashiering, and must maintain a neat and professional appearance and an organized work area.

Schedule : Monday-Friday, 8am-5pm PST

Location : 1851 N. Riverside Ave. Rialto, CA.

Primary Responsibilities :

Patient Coordination & Volume Management

Support coordination efforts for a patient population exceeding 27,000 individuals

Maintain efficient workflow and communication across clinical teams to uphold service quality

Referral Management

Process and manage incoming and outgoing referrals

Ensure timely documentation and communication for referred services

Fax Triage

Monitor and triage all incoming faxes

Route critical documentation and correspondence to appropriate parties promptly

Chart Scrubbing & Documentation Oversight

Review and scrub patient charts to ensure accuracy and completeness

Maintain up-to-date records in line with compliance standards

Regulatory Compliance & Staffing Adherence

Perform tasks within governed staffing ratios to preserve compliance integrity

Prevent risks associated with overtime and understaffing

Clinical Support Coordination

Collaborate with clinical teams while preserving their focus on core responsibilities

Avoid role absorption that would compromise care quality and operational stability

Performance & Experience Safeguarding

Maintain patient experience standards by ensuring consistent, high-quality administrative support

Safeguard throughput, regulatory outcomes, and financial performance by preventing workflow disruptions

Continuity & Stability Maintenance

Contribute to workforce consistency by supporting backfill efforts where needed

Ensure the uninterrupted execution of essential care coordination processes

Required Qualifications :

High school diploma / GED (or higher)

Current CPR / BLS certification OR willing and able to obtain within 30 days of hire

2+ years of experience in a healthcare setting (preferably in a coordination or administrative support role)

1+ years of experience handling referrals, documentation triage, chart review, and EHR systems

Preferred Qualifications :

Completion of an accredited Medical Assistant (MA) program highly desirable

Prior experience as a Medical Assistant is strongly preferred, with demonstrated clinical and clerical competencies

Proficiency in electronic health records (e.g., Epic, Cerner, Athena)

Bilingual in Spanish / English

Familiarity with medical terminology and documentation standards

Skilled in fax triage, multi-line phone systems, and care coordination platforms

Regulatory & Compliance Knowledge

Understanding of HIPAA regulations and healthcare privacy laws

Awareness of staffing ratio impacts, scheduling integrity, and documentation accuracy in regulated environments

Soft Skills :

Communication & interpersonal skills

Strong verbal and written communication, with patient-centered approach

Ability to collaborate effectively across departments and provider teams

Calm and empathetic demeanor in fast-paced, high-volume clinical settings

Organizational & problem-solving abilities

High attention to detail and time management

Adept at balancing competing priorities while maintaining service quality

Proven ability to identify workflow inefficiencies and offer practical solutions

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
