Accept and treat patients within your designated geographic areaProvide physical therapy treatments to patients in their homes as outlined in the licensed PTs plan of careMonitor and report changes in patient status to the supervising PT in a timely mannerCommunicate effectively with patients caregivers and interdisciplinary team members to ensure coordinated effective careInstruct patients family members caregivers and Home Health Aides in exercises safety awareness and use of assistive devices as directed by the supervising PTAssist the PT in updating care plans based on patient progress and response to treatmentDocument patient treatments and observations accurately and promptly in accordance with agency and regulatory standardsParticipate in case conferences and contribute to the care planning and discharge process in collaboration with the PTAdhere to all federal state and local regulations as well as agency policies and professional standards of practiceQualifications :
Physical Therapist Assistant Program GraduateLicensed Physical Therapist Assistant in the state of New York or eligible.Mandatory Certifications : First Aid AED & CPR Certifications.Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.Ability to travel to patients homes within the assigned geographic territoryRemote Work : No
