Job Summary :

Spring Creek Healthcare Center, a premier 179-bed nursing home located in Perth Amboy, NJ, is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. As a Social Worker at Spring Creek, you will play a vital role in providing exceptional care to our patients and residents, while promoting their physical, emotional, and social well-being.

Responsibilities :

Conduct initial and ongoing assessments of patients' and residents' social, emotional, and psychological needs

Develop and implement individualized care plans to address identified needs and goals

Collaborate with the interdisciplinary team to ensure comprehensive care and promote positive outcomes

Provide counseling and support to patients, residents, and their families to address grief, loss, and other emotional challenges

Facilitate communication between patients, residents, and healthcare providers to ensure timely and effective care

Identify and address potential barriers to care, such as cultural or linguistic differences

Participate in quality improvement initiatives to enhance patient and resident care and satisfaction

Maintain accurate and detailed records of patient and resident interactions and outcomes

Assist in the grievance proccess
