Nurse Practitioner, Palliative Care, Per Diem – Santa Rosa, CA

Providence Medical Group in conjunction with Santa Rosa Memorial Hospital is seeking an inpatient Palliative Care Nurse Practitioner to join their team in Santa Rosa, California. This is a per diem position with a tenured, passionate and collaborative group that works closely together. The role involves providing consultative services and collaborating with hospitalists, intensivists and specialty services for patient care in high-needs communities with many acute cases from facilities to include Santa Rosa Memorial, Petaluma Valley Hospital and Healdsburg Hospital. The regional team, which includes Napa, Sonoma, and Eureka, offers extensive support. The ideal candidate is a strong team player with experience working with interdisciplinary teams, comfortable with independent practice, and is an excellent communicator.
