Nurse Referral Coordinator - Presbyterian Heart and Vascular Care

Overview

Presbyterian is seeking a Nurse Referral Coordinator to provide direct patient care to an assigned group of patients in an Outpatient Clinic . Common responsibilities include facilitating and following the cardiac patient from point of referral and then coordinating arrangements for referral care, procedural care and / or long-term cardiac care. The navigator is the contact person for the patient / family and serves as the liaison to all physicians and Healthcare staff providing care and support for the patient.

Type of Opportunity : Full Time (1.0 FTE / 40 hours per week)

Work Schedule : Monday - Friday
