Field RN - Home Health Visits

Activa Home Health (an AccordCare Company) - a licensed, Medicare-certified home health agency - is seeking experienced and compassionate Field Registered Nurses (RNs) to join our team on a part-time basis.

Position Summary

The Field RN is responsible for providing high-quality nursing care in accordance with each patient's individualized Plan of Care . This role ensures that all patients receive care focused on safety, well-being, and satisfaction.

Experience with WellSky / Kinnser and prior home health experience are a plus.

Key Responsibilities

Deliver safe, effective, and compassionate care to patients following the individualized Plan of Care .

Communicate promptly with the care team regarding changes in patient condition, ensuring effective case management.

Provide clinical care such as :

Medication administration

Wound care

Infusions

Catheter care

Transferring and positioning patients safely

Collaborate closely with the Nurse Supervisor and Manager of Clinical Practice to ensure care aligns with agency standards.

Maintain thorough, accurate documentation in the electronic health record (EHR) .

Demonstrate empathy, professionalism, attention to detail, and a proactive approach to patient care and problem-solving.

Required Qualifications

Current RN license in the State of Florida

Wound care skills required

Strong communication, teamwork, and customer service abilities

Proficiency in using electronic health record systems ,Oasis experience for documentation and communication required

Minimum 1 year of home health experience (preferred)

Alzheimer CEU approved by DOEA & HIV / AIDS CEUS (preferred)

1 years : CPR card (required)

1 years : Home Health Nurse (required)
