Resident Care Coordinator (RCC)

Job Summary :

The Resident Care Coordinator (RCC) is a vital member of our Care Team responsible for the delivery and oversight of personal care and related services to residents in the community. The RCC ensures that services are provided in accordance with the documented Care Plans (CP) and supports the overall well-being of our addition the RCC plays a key role in training supervising and motivating Caregivers Medication Technicians and other support staff while maintaining a positive and enriching environment for both residents and staff.

Key Responsibilities :

  • Resident Care Oversight : Implement and monitor personal care services as outlined in each residents Care Plan (CP) ensuring high-quality care and safety.
  • Supervision and Training : Assist in the recruitment training and supervision of Caregivers and Medication Technicians promoting best practices in resident care and ensuring compliance with established policies and procedures.
  • Team Leadership : Foster a culture of teamwork and collaboration among care staff encouraging professional development and motivation.
  • Communication : Serve as a liaison between residents families and healthcare providers to ensure clear communication regarding resident needs and care plans.
  • Documentation : Maintain accurate and timely documentation of resident care activities updates to Care Plans and staff performance as required.
  • Quality Assurance : Participate in regular assessments of resident care and staff performance to ensure service quality and adherence to regulations and standards.
  • Emergency Response : Be prepared to respond to medical emergencies and coordinate care with healthcare professionals as necessary.
  • Community Engagement : Encourage and participate in social and recreational activities for residents to promote engagement and quality of life within the community.

Qualifications :

  • High school diploma or equivalent; further education or certification in healthcare preferred (e.g. CNA HHA or equivalent).
  • Previous experience in a healthcare or residential care setting with a focus on elder care preferred.
  • Strong understanding of personal care practices and care planning.
  • Exceptional communication and interpersonal skills.
  • Ability to train and motivate a team fostering a supportive work environment.
  • Strong organizational skills and attention to detail.
  • Ability to handle sensitive situations with compassion and professionalism.
  • First Aid and CPR certification is a plus.
  • Working Conditions :

  • May require working evenings weekends or holidays to meet resident needs.
  • Ability to lift carry and assist residents as needed.
  • Work in a residential healthcare setting with a team of caregivers and healthcare professionals.
