In this vital role, you'll be a guiding force for families navigating complex systems-connecting them to the services and supports that improve health, stability, and overall well-being. You'll build trusted relationships, coordinate care, and ensure that every intervention is both effective and compassionate.

Responsibilities

• Coordinate and oversee services for your assigned caseload
• Build strong, supportive relationships with families and service providers
• Connect individuals to essential resources such as housing, financial assistance, and substance use services
• Track outcomes and ensure interventions are impactful and client-centered
• Advocate for families and help them access the tools they need to succeed

Education and Experience

Bachelor Degree in Human Service or related field with two years of experience or Registered Nurse or Licensed Practical Nurse with one year of experience. Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and / or other systems as well as those receiving preventive services. Experience providing service coordination and information, linkages, and referrals for community-based services.
