Job Description

The Social Worker Assistant collaborates with the interdisciplinary team to promote and protect resident rights and the well-being of all patients / residents. This includes educating staff on the role of the social services

The Social Worker Assistant participates in the development of a written, interdisciplinary plan of care for each patient / resident to address social services needs

The Social Worker Assistant hold meetings with patients / residents, families, and significant others as requested, to provide support and information to improve quality of life

The Social Worker Assistant performs patient / resident discharge planning activities such as family / patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.

The Social Worker Assistant establishes relationships and maintains contact and referral flow with community based agencies / services
