SUMMARY

Provide personal care under the supervision of a nurse according to the patient's plan of care.

Collaborate with transdisciplinary team in a variety of settings.

PRIMARY FUNCTIONS

  • Provide personal care and assist with ADL's according to the Plan of Care - POC.
  • Provide safe environment for patients.
  • Safely ambulate / transfer patient as appropriate.
  • Assist with passive and active ROM exercises as directed by the nurse.
  • Prepare nutritional snacks as appropriate for the patient.
  • Under the direction of the nurse, perform simple wound care, skin care, and bowel care

protocols according to the plan of care where appropriate.

  • Recognize and promptly report any changes in the mental or physical status of the
  • patient to the nurse.

  • Record in clinical record, on appropriate documents, the activities provided and
  • observations of patient at the time of visit.

  • Complete all charting, timesheets and mileage logs as defined by the agency policy and
  • procedures.

  • Attend and actively contribute during patient care conference.
  • Participate in HNA discipline meetings, team meetings, staff meetings, retreats and inservices as appropriate.
  • Attend in-services to obtain 3 education hours per quarter.(need 12 a year)
  • Participate in the on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with other designated activities as necessary (scheduling, supply
  • organization, answering office telephone, etc.)

  • Demonstrate positive customer-service responsiveness according to agency standards.
  • Flexible in meeting patient / family needs that require variable shifts and weekends.
  • Participate in chart review quality improvement activities and other meetings as
  • assigned.

  • Participate in coordination of and communication with volunteer staff.
  • Provide basic caregiving education to caregiver / family members, such as turning and
  • positioning, transfer techniques, and changing linens

  • Complete annual competency requirements.
  • SUPERVISORY RESPONSIBILITIES

    NONE

    QUALIFICATIONS (EDUCATION and / or EXPERIENCE)

    Must be state tested nursing assistant in good standing and have 12 months of experience

    as a nursing assistant.

    LANGUAGE SKILLS

    Must be able to read and write English.

    Ability to read and interpret documents such as safety rules, operating and maintenance

    instructions, and procedure manuals. Ability to write routine reports.

    REASONING ABILITY

    Ability to apply common sense understanding to carry out instructions furnished in written,

    oral, or diagram form. Ability to deal with problems involving several concrete variables in

    standardized situations.

    CERTIFICATES, LICENSES, REGISTRATIONS

    Must be a Florida Certified Nursing Aide-CNA.

    PHYSICAL DEMANDS

    pounds while utilizing proper body mechanics and appropriate mechanical lifting devices

    provided. Must be able to lift with the use of lift devices or assistance for residents with weight

    in excess of 100 pounds.

    Specific vision abilities required by this job include close vision, distance vision, color vision,

    peripheral vision, depth perception and ability to adjust focus.

    WORK ENVIRONMENT

    While performing the duties of this job, the employee is occasionally exposed to wet or humid

    non-weather conditions, working near mechanical parts, and toxic or caustic chemicals. Regular

    exposure to fumes or airborne particles.
