Position Summary :   POD Health is seeking a Licensed Clinical Social Worker (LCSW) to join our clinical team full-time, on-site in Brooklyn, NY. In this role, you’ll support patients by assessing mental health needs, developing care plans, and collaborating with a multidisciplinary team to ensure well-rounded support for conditions such as addiction, depression, and anxiety.

Key Responsibilities :

• Conduct thorough biopsychosocial assessments and develop personalized care plans.
• Focus on treating addiction, depression, and anxiety, primarily in senior populations.
• Liaise closely with the medical team to define care goals and manage patient outcomes.
• Educate patients about available services and promote active participation in their care.
• Maintain timely and accurate documentation in the Electronic Health Record (EHR).
• Participate in weekly clinical supervision and interdisciplinary team meetings.
• Uphold NASW ethical and professional standards in all areas of practice.
• Build therapeutic rapport with patients from diverse backgrounds while maintaining clinical boundaries.
