Job Description

Job Title : Registered Nurse

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty

License : Must have license in hand, no exceptions

Certifications : CPR, BLS

Must-Have : Proficiency in HomeCare HomeBase and OASIS, used within the last 6 months or completion of a retraining course

Job Description :

• Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process
• Responsible to Clinical Manager; directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses
• Perform home visits and attend periodic office meetings
• Must have reliable transportation, a current driver's license, and agency-required liability insurance
• Handle an average of 5-8 patients per day
• Manage various types of cases, including start of care, wound care, and follow-up visits
• Required to travel up to 75 miles from the home clinic, mileage reimbursed at IRS Standard rate
• Must complete all documentation within 24 hours of the patient appointment; delayed or incomplete documentation may lead to cancellation
• On-call duties vary by location and profession, to be discussed during the interview
• Local applicants are accepted at the same rate
• Must be open to travel between 150-200 miles per week and cover VA Beach and Norfolk areas
