We are looking for a PRN Occupational Therapist (OT) to join our committed rehabilitation team. This role is ideal for a skilled therapist who seeks the flexibility of PRN scheduling while making a meaningful impact. The position requires on-site presence.

Position Summary :

As a PRN Occupational Therapist, you will evaluate and treat residents to enhance their ability to perform daily living activities, increase their independence, and achieve rehabilitation goals. You will work collaboratively with a multidisciplinary team to ensure high-quality, person-centered care.

Key Responsibilities :

• Conduct comprehensive evaluations of residents' physical, cognitive, and psychosocial needs.
• Develop and implement individualized treatment plans aimed at improving residents' functional abilities.
• Provide therapeutic interventions, including adaptive technique training and assistance with assistive devices.
• Monitor residents' progress and make necessary adjustments to therapy plans to achieve set outcomes.
• Collaborate with the care team, residents, and families to identify and support therapy goals.
• Educate residents and caregivers on therapeutic strategies and techniques to promote independence.
• Maintain accurate documentation of therapy services, evaluations, and progress notes.
• Comply with all state and federal regulations, as well as facility policies and procedures.
