Job Description

We are seeking a part-time LICSW Clinical Social Worker specializing in geriatrics for a daytime role in Minneapolis, MN.

• Provide psycho-social evaluations, diagnoses, and treatment for nursing home residents, including individual, group, and family therapy.
• Collaborate with interdisciplinary teams to ensure quality care and continuity.
• Manage 10-15 patient contacts daily with flexibility for scheduling.
• Utilize electronic medical records and adhere to Medicare documentation standards.
• Qualifications include LICSW licensure, 1+ years of clinical experience, and a passion for working with older adults.
• Preferred : experience in nursing home mental health and Medicare guidelines.
