Essential Duties And Responsibilities
Maintain accurate and up-to-date individual case management and data records for Trueblood referrals and enrolled participants within FBH's EMR.
Adhere to all internal policies and procedures related to data management, privacy, and security, ensuring compliance with applicable federal and state regulations.
Serve as primary data entry person for assigned diversion programs and ensure documentation is completed on time and kept up to date so accurate data can be transmitted based on contract submission requirements. For applicable programs, receive and submit required data daily into the HCA Program Data Acquisition Management System (PDAMS) data capture system by the 20th of each month. Timely and complete data entry is essential for program monitoring, compliance, and evaluation purposes.
Respond to data fixes and corrective actions within twenty (20) business days of receiving the information. If errors or inconsistencies are identified by HCA or through internal data reviews, the Data Specialist must implement necessary corrections and provide documentation of resolution within the designated timeframe.
Attend mandatory PDAMS training sessions as directed by the HCA Contract Manager. Follow PDAMS data entry guidelines and requirements, as outlined in the PDAMS training manual. Ensure proficiency in PDAMS reporting requirements are maintained and implement best practices to maintain data accuracy and integrity.
For programs that apply, discharge inactive participants from the program within sixty (60) calendar days of no contact unless they are reincarcerated, enter treatment, or are hospitalized, then the time frame for discharge is ninety (90) calendar days, ensuring accurate reflection of current caseloads and service engagement. This includes processing program exits in accordance with established program policies and guidelines.
Submit all required program documentation, including program enrollment, program end, activity participation, and housing-related forms within a month of the time of the activity occurring to ensure compliance with program reporting requirements.
Regularly review data reports to identify and correct potential errors. This includes verifying accuracy in participant records, service activities, and program outcomes. Any discrepancies must be promptly addressed, with data corrections submitted as necessary.
Complete and submit qualitative quarterly reports with comprehensive and detailed information by assigned deadlines. Reports shall include, but are not limited to, participant progress, service delivery updates, program challenges, and best practices to enhance service effectiveness.
Participate in an internal quality assurance process to ensure data completeness, accuracy, and consistency. This may include routine audits, staff training, and corrective action plans as needed to enhance reporting reliability.
Facilitate workflow between program staff, data entry processes and staff, and long-term storage of documents within FBH's EMR, and other identified areas.
Meet identified key performance indicators and metrics for the position as identified for the programs, division and agency.
Maintain productive working relationships with other staff to ensure efficient and seamless coordination of client care and business needs.
Partnering as a team with Compliance and IT staff to deliver current programming.
Maintain internal FBH reporting information and provide it to Director and PAs, highlighting trends and whether performance metrics are being met.

Qualified Applicants Will Have :
Required :
High School Degree.
6 months of paid work experience in data management.
Preferred :
One year of experience working with data systems.
Knowledge, Skills & Abilities :
Must possess excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.
Demonstrated ability to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude, and empathy.
Knowledge and skill in use of common software, including the Microsoft Suite, with moderate to advanced knowledge of Excel.
Physical, Sensory & Environmental :
Work is generally performed in an office setting which may involve sitting and use of computers for four to eight hours per day.
May be required to lift 10 - 20 pounds.
Work may be performed out of office and travel may be needed to outside areas.
Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.
Pay Range :
21.33- $25.89) (Hourly / Non-Exempt) : All pay is based on longevity at Frontier Behavioral Health; thus, the starting base pay for this position is $21.33 per hour.
