Job Description :

We are seeking a compassionate Physical Therapist to join our home health team. This role focuses on delivering personalized physical therapy in patients homes to help them improve mobility strength and overall function.

Responsibilities :

Assess patients physical condition and mobility

Develop and implement individualized treatment plans

Provide physical therapy services based on best practices

Educate patients and families on home exercises and techniques

Collaborate with other healthcare professionals for coordinated care

Maintain accurate patient records and documentation

Qualifications :

Licensed Physical Therapist in the state of Texas

Strong understanding of physical therapy principles

Excellent communication and interpersonal skills

Ability to work independently with a flexible schedule

Empathetic and patient-focused approach

Home health experience preferred
