Case Manager-Rhode Island-Local Travel Required

What You'll Do :

• Manage a caseload and conduct outreach (telephonic & community-based)
• Complete assessments in-home or in the community
• Coordinate medical services and referrals
• Assist participants in accessing social services and making appointments
• Conduct off-site visits to homes, hospitals, and community agencies
• Collaborate with interdisciplinary teams and contribute to program improvements
• Maintain accurate documentation in EMR
• Participate in outreach and marketing activities
