RN Care Manager - Full-Time - Day Shift

Community Care is hiring a RN Care Manager to join our PACE Program team. RN Care Manager Job Responsibilities :

• Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w / families and other supports for additional information as needed.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality / coordination and effectiveness of services.
• Complete all required documentation and charting and assists in the development and maintenance of member records.
