About The Role

This is a full or part-time position that offers flexible scheduling. The 6 : 00 am-6 : 00 pm hours are ideal to accommodate the various scheduling needs of patients and their families.

Job Description

Under the Director of Nursing's guidance, the LPN nurse promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members, providing physical and psychological support to patients, friends, and families; and supervising assigned team members.

RESPONSIBILITIES

• Assists in carrying out rehabilitation programming and activities for patients, such as the development of social skills, development of daily living skills
• Provides personal care services to patients including oral hygiene, bathing and hair care as needed, nail care, diapering, and dressing
• Provides bed care to patients, repositioning, ambulating, and assisting with elimination needs. Moving to and from wheelchairs, etc.
• Administers medications to patients orally, intra-, or via feeding tube subcutaneously
• Monitors effects of medications records medication is given and patients' reactions
• Observe and monitor the movement and activities of patients in their home
• Assists / reviews MAR monthly and as needed to ensure accuracy
• Applicants must be able to travel .
