Responsibilities

To supervise and direct the oncology patient nurse navigator program. Additionally manages the care of identified patients with specific cancer diagnoses in collaboration with an interdisciplinary team to ensure high quality care across all care delivery sites. Serves as a single point of contact for patients caregivers and referring physicians to provide resources and assistance with accessing clinical and supportive care services offered with in Northside Hospital and the community.

Qualifications

REQUIRED :

1. Bachelors of Nursing Degree.

2. (3) Three years of clinical experience in oncology.

3. Demonstrated ability to work independently in a self-directed environment.

PREFERRED :

1. Previous experience in project management education or case management. 2. Masters of Science Degree in Nursing 3. Previous experience working with culturally diverse populations. 4. Possess strong working knowledge of medical systems and community resources. 5. Able to establish strong working relationships within a medical system.
