\n Pay Rate : $55 / hour

Shift Requirements : \n\n \n\t
Day Shift : 8 : 00 AM – 5 : 00 PM
\n\t\n\t
No on-call, weekends, or overtime required
\n\t\n\t
Travel required within the Colorado Springs area
\n\t\n\t
Mileage reimbursement : $0.45 per mile
\n\t\n
\n\n
Preferred Experience :
\n\n \n\t
HCHB (Homecare Homebase)
\n\t\n\t
Hospice or Palliative Care experience
\n\t\n
\n\n
Licensure & Requirements :
\n\n \n\t
Active Colorado RN License or Multi-State Compact License
\n\t\n\t
Reliable transportation and valid auto insurance
\n\t\n
\n\n\n Position Summary : \n\n
The Registered Nurse – Case Manager is responsible for providing comprehensive assessment and coordination of care for high-risk members receiving hospice or palliative services. This role focuses on ensuring quality outcomes through individualized care planning, collaboration with interdisciplinary teams, and proactive management of member needs.
\n\n\n Primary Responsibilities : \n\n \n\t
Perform comprehensive assessments of high-risk members and develop individualized care plans.
\n\t\n\t
Collaborate with primary care providers, social workers, and other healthcare professionals to ensure implementation of a comprehensive care plan.
\n\t\n\t
Identify and address cost-efficient and appropriate levels of care, ensuring all services align with patient needs and goals.
\n\t\n\t
Facilitate coordination of non-medical supports such as housing, transportation, or community resources.
\n\t\n\t
Manage care transitions to promote continuity and prevent readmissions.
\n\t\n\t
Provide education and support to patients and families regarding disease processes and treatment options.
\n\t\n\t
Maintain accurate and timely clinical documentation and case management records.
\n\t\n\t
Identify opportunities for health promotion and illness prevention.
\n\t\n\t
Act as a patient advocate, maintaining strict confidentiality and upholding professional and ethical standards.
\n\t\n
\n\n\n Qualifications : \n\n \n\t
Associate’s or Bachelor’s degree in Nursing (BSN preferred).
\n\t\n\t
Current, unrestricted Registered Nurse (RN) license.
\n\t\n\t
Case Management Certification (preferred).
\n\t\n\t
Strong analytical and critical thinking skills.
\n\t\n\t
Excellent communication, organization, and documentation abilities.
\n\t\n\t

IDR Job ID #472746. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
