Summary : Monitor service delivery of caseload
Position Type : Full Time, Non-Exempt
Essential Functions :
- Review referrals and assess program eligibility
- Complete intake and initial assessment of services timely
- Adheres to all of the OAAS policies, procedures and timelines
- Develop and monitor service plans in accordance with regulatory guidelines
- Maintain contact with program participants in accordance with regulatory guidelines
- Assist with developing and monitoring monthly budgets, where applicable, for program
Participants
Assist program participants with obtaining community support, resources and servicesMaintain required documentation (service logs, POCs, etc)Submit service logs and billing documentation according to procedureAttend all OAAS Mandatory meetings and trainingsCheck and respond to all phone calls dailyMaintain training requirementsReport any significant concerns and critical incidents to supervisor / SIMSRotate on call 24 hour support to program participantsCompetencies

Required Education and Experience : Bachelor’s degree in the Human Service Field. Within first three months of employment, must become certified in IHC and Plan of Care. Maintain valid driver’s license and vehicle insurance, excellent writing and computer skills required.
