By coordinating the complexities of each unique care journey, Porter helps close the gaps with the largest impact on quality measures, total cost of care, risk adjustment, and member experience.

What You'll Do

The Care Coordination Manager leads a team of Care Guides to ensure high-quality, person-oriented care for members navigating complex health and social needs. This role is responsible for operational oversight, regulatory compliance, team development, training, and strategic alignment with organizational goals.

Key Responsibilities :

Team leadership & Supervision

• Supervise and mentor Care Guide Team
• Create high-performance team culture and drive team to achieve all KPIs
• Manage schedules, caseloads, performance monitoring, and evaluations.
• Foster a culture of accountability, innovation, and collaboration
• Provide training on workflows / policies, member engagement and communication strategies

Quality & Compliance

Qualifications :

Bachelor's degree in nursing, Social Work, Case Management, or related field, or equivalent experience

Minimum 3-5 years of experience working in healthcare leading and managing teams.

Exceptional communication, collaboration, and critical thinking skills

Experience working in healthcare IT systems / care management software / Electronic Health Records
