Description

Salary range : $70,000 - $80,000 annually

As the Nurse Case Manager, you will work one-on-one with clients to create customized care that will be delivered by your nursing and aide staff. With your oversight and training, clients will receive amazing care in the comfort of their home. You will truly make a difference every day as you work alongside a supportive team. The administrative component of this job consists of internal case management, start of cares and supervisory visits, and aide orientation and ongoing trainings. The field-based aspect of this position allows you to spend quality time with field staff in client homes providing professional guidance.

Responsibilities

  • Perform case management including but not limited to initial intake communications, management of authorizations, communication with referral sources, oversight, and coordination of care.
  • Ensure staff competencies consistent with agency policy and procedures.
  • Perform clinical supervision of patient care consistent with standard nursing principles.
  • Perform supervisory nursing visits.
  • Ensure services are compliant with professional standards, state and federal regulatory requirements.

Qualifications

  • Current RN Professional license - state of Florida is required.
  • Minimum of one year experience within home care preferred.
  • Must be able to travel to clients' homes as needed.
  • Must have reliable transportation and a valid driver's license.
  • Must have excellent time management skills to prioritize and organize workload.
  • Positive attitude and strong work ethic.
  • Excellent communication and customer service skills.
  • Experience with Matrix / AlayaCare a Plus!
