Scope of Role :

Home health LPTA - will be an active member of an interdisciplinary team support treatment for addressing the skilled and medically necessary needs of patients and provide discipline therapist guided interventions to improve the safety and functional independence of its clients through the rehab process. Feedback from the LPTA is critical to determine and recommend the patient's progression through the continuum of care.

Must have at least 1-year Home Health experience ability to use laptop for documentation and cell phone for communication.

Patient Ration : 6-7 patients per day
