Overview
In this role, you will coordinate and deliver exceptional physical therapy services tailored to individual patients and groups, aligning with departmental objectives and the principles of physical therapy.
Key Responsibilities :
- Conduct evaluations of assigned patients by collecting data from various sources, including patient records, interviews, observations, and collaboration with team members.
- Create and implement personalized treatment programs, discussing the plan of care with patients and educating families and caregivers on home exercises as needed.
- Utilize activities and strategies to promote health and achieve treatment goals, continually revising intervention plans based on patient evaluations.
- Administer direct therapy services in various settings, customizing therapy plans to meet patients' physical, emotional, and intellectual needs as part of a comprehensive care plan.
- Document patient evaluations, treatment plans, progress notes, and other essential reports per professional standards.
- Communicate effectively with patients, families, physicians, and interdisciplinary team members regarding patient status and overall care.
- Build rapport and collaborate with interdisciplinary team members and families throughout the intervention process.
- Engage in professional development through educational programs, literature, and in-service training.
- Supervise physical therapy assistants, techs, and students, ensuring safe and effective patient care and departmental operations.
- Stay informed on the proper use of patient care equipment.
Essential Qualifications :
Ability to work specified hours and perform all essential job functions, including effective communication and visual acuity.Education and Certification :
An active Physical Therapist license in the state of practice upon hire is required.Expectations :
