Summary of Duties

As a Family Coach (Human Service Case Manager) Bilingual Vietnamese Required you will :

• Conduct interviews to determine family needs and eligibility and complete program applications for program benefits and if eligible issues appropriate support.
• Engage clients in family evaluation of strengths and jointly develop a case plan and identify the steps the family will take towards self-sufficiency and meeting their basic needs.
• Monitor progress needs and makes necessary modifications to the case plan.
• Initiate and participate in case staffing employment readiness life skills training and supports job search as needed.
• Assist with crisis and safety planning for individuals experiencing domestic violence.
• Explore alternate resources and link clients to community services and may also assist with supporting resources and services within the community.
• Bilingual Vietnamese is Required
