Halcyon Home is hiring a Home Health Physical Therapist Assistant PRN Per Visit to support our growing geriatric home health population in Northwest Austin TX and surrounding areas. We offer competitive pay and wonderful team support.

Check out the best places to work link! a Physical Therapist you will provide physical therapy to our patients in both community and residential settings in the Northwest Austin area . A minimum of one year of home health experience is preferred along with a proven ability to manage a fast-paced independent work schedule. Knowledge of Homecare Homebase software is a plus!

Essential Functions :

• Performs ongoing clinical assessment including OASIS assessments at appropriate time points to identify patient needs determine level of functioning and appropriateness of patient for home care.
• Develops and evaluates the plan of care in partnership with the patient representative (if any) and caregiver(s).
• Documents prepares and completes clinical notes phone orders and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
• Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members when applicable in the performance of patient care that is consistent with patients needs.
• Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Provides supervision and guidance to physical therapy assistants and home health aides as appropriate.
• Plans and provides appropriate treatment for neuromuscular neurological and orthopedic disorders based on medical referral and therapy assessment.
• Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Meets mandatory continuing education requirements of the Agency and licensing board.
• Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers wheelchair canes a safe manner. Utilizes appropriate therapy equipment specific to patients diagnoses and needs.
• Provides patient caregiver and family counseling; Provides patient and caregiver education.

Compensation based on experience and productivity.
