Job Description

Job Title : Registered Nurse

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience required

License : Must have a valid nursing license prior to starting

Certifications : CPR, BLS

Must-Have :

  • Reliable transportation
  • Current driver's license
  • Agency-required liability insurance
  • Proficiency in HomeCare HomeBase EMR
  • Experience in OASIS assessment within the last 6 months
  • Willingness to travel up to 75 miles from the home clinic

Job Description :

  • Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
  • Responsible to the Clinical Manager.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
  • Conduct home visits and attend periodic office meetings.
  • Manage an average of 5-8 patients per day, achieving 6-8 productivity points daily.
  • Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits.
  • Complete all patient documentation within 24 hours of each appointment.
  • Participate in on-call rotations, as required.
  • Attend weekly meetings at the clinic and accept schedule assignments.
  • Report to designated work location.
  • Maintain a high standard of patient care while traveling an average of 200 miles weekly within the assigned territory.
