About the Role

Corus Health is seeking a Community Liaison to join our Albuquerque-based team, supporting both our Home Health and Hospice service lines. This is a high-impact role for a relationship-builder who thrives in the post-acute care space and wants to help shape the future of care in our community.

Key Responsibilities

• Promote awareness and appropriate utilization of Corus Health's home health and hospice services.
• Build and maintain referral relationships with hospitals, SNFs, ALFs, physician practices, and other post-acute partners.
• Coordinate referrals and provide timely follow-up to ensure smooth transitions of care.
• Educate healthcare professionals and the public about the hospice Medicare benefit and in-home services.
• Collaborate closely with clinical and operational teams to ensure alignment and excellence in service delivery.
• Support the development of marketing strategies to achieve admission and census goals.
• Represent Corus Health professionally at community events, in-service trainings, and outreach initiatives.
• Analyze referral trends and provide strategic feedback to leadership.
• Maintain compliance with HIPAA and all relevant regulations.
