Become the Director of Case Management You Aspire to Be

Oversee the daily operations and human resources management of the Case Management department.

Ensure the interdisciplinary plan of care and discharge planning process effectively meet patient care outcomes and key care indicators.

Act as a patient and family advocate, ensuring service delivery meets their needs.

Provide guidance and support to Case Managers and staff, ensuring high-quality operations and adherence to organizational standards.

Develop strategic relationships with insurance companies, self-insured employers, and other healthcare networks.

Qualifications

Preferred current CCM® or ACMTM certification.

Must independently complete assessments within your discipline's scope of practice.

If licensure is required in the state, maintain an active license.

Nursing candidates must possess a bachelor’s degree in nursing (BSN) with RN licensure.

Other eligible healthcare professionals must have a minimum bachelor's degree; a graduate degree is preferred.

Minimum three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.

Willingness to work weekdays, weekends, evenings, nights, and potentially holidays.
