Nurse Practitioner- Geriatrics

Private geriatrics practice providing home and facility care across eastern Massachusetts (Worcester, Middlesex, Essex and Suffolk / Norwalk counties OR Bristol, Plymouth, Barnstable and Suffolk / Norwalk) is adding a Nurse Practitioner to their team. NP will see 10-25 patients per day depending on locations / facilities and function autonomously. Part time position- 3 days per week, 24 hours per week. Scribe / dictation will be provided for each visit to assist the NP. Nurse Practitioner will be expected to provide coordinated care services between the patient, family, physicians, additional providers, and other care team members. Experience as a Family Nurse Practitioner or Adult Geriatric Nurse Practitioner is highly preferred, though RN experience in a similar setting will be considered.

Candidate must demonstrate proficiency in : clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients. Experience with dialysis patients, cardiac and diabetes education is a plus.

No weekends, rare phone call. Applicants must have reliable transportation and be comfortable making home visits. Mileage reimbursement is offered.
