Pennsylvania Licensed Behavioral Health Case Manager

A company is looking for a Case Manager Behavioral Health.

Key Responsibilities :

Engages members telephonically to identify barriers to care related to their Social Determinants of Health

Facilitates the development of care plans by identifying members' strengths, needs, and goals, providing follow-up as needed

Acts as a liaison within the member's health care team, especially in complex behavioral health situations

Qualifications :

Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required

Minimum of 3-5 years post licensure experience in an integrated care or healthcare setting

Strong understanding of Social Determinants of Health and Motivational Interviewing techniques

Ability to work independently and resolve complex behavioral, medical, and psychosocial issues

Exceptional assessment and engagement skills, with proficiency in telephonic and virtual communication
