Job Description

Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc . “The Heart of Our Community” provides comprehensive services including adult / pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult / pediatric dental care.

RN Care Manager (C3) Summary :

As an integral member of the care management team, the RN Care Manager (RN CM) will have the opportunity to have a profound impact on the lives of people living with complex and / or chronic conditions, many of whom also face multiple barriers in their lives, which make it difficult for them to achieve the self-care required to improve their health and well-being. This position is currently hybrid, but requires flexibility, and may vary from day-to-day to meet members where they are. Outreach methods are based on the needs of the organization and the member, and may include telephonic, or in-person engagements in a variety of potential settings, such as the health center / practice, community, home, or an inpatient facility. The RN CM is under the general supervision of the Director of Nursing.

Schedule :

Mon – Fri : 8 : 30 am – 5 : 00 pm

Pay Range :

$81,300 - $88,000 annually

Required Qualifications :

• Experience within the ACO’s member population preferred, including Medicare / Medicaid member populations.
• Experience working with Federally Qualified Health Centers / Primary Care Provider practices is strongly preferred.
• License to practice as an RN from the Commonwealth of Massachusetts required.
• 3-5 years of nursing experience; home health care, ambulatory care, community public health or Case / Care Management.
• A valid driver's license and provision of a working vehicle.
• Experience working with patients with chronic medical and behavioral health needs preferred.
• Maintain current certification in CPR at the healthcare provider level.
• Bi-lingual in Spanish or Portuguese preferred.
• Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.
