We’re looking for a Clinical Coordinator with SMI experience to provide clinical and administrative support and supervision.
DUTIES AND RESPONSIBILITES
Staff Supervision & Training : Supervises, trains and mentors Case Managers and Peer Support team to ensure clinical quality standards and productivity goals are met.
Program Coordination : Assist the Chief Clinical Officer in implementing and evaluating mental health programs, including programs specifically designed for individuals with SMI. Ensures these programs are aligned with best practices, clinical guidelines, and regulatory requirements
Coordination of Services : Facilitate access to a variety of services, including therapy, medication management, and support groups. Ensure continuity of care among different providers.
Compliance and Reporting : Maintain accurate and up-to-date records of patient interactions, treatment plans, and progress notes in compliance with regulatory standards. Maintain and complete monthly deliverables for compliance with Mercy Care requirements and guidelines.
Communication : Serve as a liaison between patients, families, and healthcare providers to ensure everyone is informed and involved in the care process.
Crisis Intervention : Respond to behavioral health crises, providing immediate support and intervention when necessary.
Education : Educate patients and their families about mental health conditions, treatment options, and coping strategies.
Advocacy : Advocate for patients’ needs and rights within the healthcare system and the community.
Collaboration : Work with interdisciplinary teams, including psychiatrists, social workers, and other mental health professionals, to provide holistic care.
Other additional related job duties may be assigned as required to meet the needs of the service participants / patients / clients.
Requirements :
KNOWLEDGE, SKILLS, AND ABILITIES
Bachelor’s degree in social work, psychology or a related field 2-year's experience of social work, psychology, and / or case management work. An equivalent combination of education, experience, and training will be considered.
Experience working with the SMI population.
Experience in a lead or supervisory role preferred.
Proficiency using software and technology to include Microsoft 365 (Office, Teams, Excel, etc) and EMR / EHR (NexGen preferred).
Ability to maintain strict adherence to HIPAA and demonstrates ability to handle sensitive situations with discretion and empathy.
Valid driver’s license and verification of insurance required.
Current CPR / First Aid certifications or ability to obtain within 30 days of hire.
Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire.
Strong communication and organizational skills.
Knowledge of relevant laws, regulations, and best practices.
