Schedule : Monday – Friday, 8 : 00 a.m. – 5 : 00 p.m.

Salary Range : $66,000 – $80,000

Position Summary

We’re seeking an Associate Clinical Social Worker (ASW / ACSW) to join our IBH team in Berkeley, CA. This position is ideal for early-career clinicians looking to strengthen their experience in integrated care while receiving clinical supervision. You’ll play a central role in connecting clients to essential behavioral, medical, and social services.

Key Responsibilities

Care Coordination & Program Support

• Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs
• Conduct comprehensive client assessments and contribute to individualized care planning
• Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources
• Monitor engagement and support follow-up care to maintain service continuity
• Collaborate with internal care teams and external partners to address client needs holistically
• Participate in multidisciplinary team meetings, case reviews, and service planning sessions
• Document services and progress accurately in the electronic health record (EHR)
• Assist with data collection and service tracking to meet reporting requirements
