Location : Mishawaka, IN (Travel required within surrounding counties)

Schedule : 2 set days per week (no weekends or holidays required)

Pay Rate : Flat $30 / hour

Position Summary :

We are seeking aPart-Time Licensed Practical Nurse (LPN)to join our IHCC team. This role is responsible forproviding care coordination, client visits, and ongoing supportfor members enrolled in the IHCC program. The LPN will be scheduledtwo fixed days each weekto ensure consistency with documentation, client follow-ups, and case management.

Key Responsibilities :

• Completein-home and telephonic visitswith IHCC members in compliance with Indiana Medicaid guidelines.
• Assist withdeveloping, updating, and reviewing care plansbased on member needs.
• Document all encounters in AxisCare and ensureEVV-compliance.
• Conduct health and safety checks, review medication adherence, and support family / caregiver education.
• Communicate with case managers, primary care providers, and other professionals involved in the members care.
• Track assigned monthly IHCC hours to ensure members receive the full scope of approved services.
• Provide recommendations to the Director and Assistant Director on client progress or concerns.
• Maintain accurate, timely, and professional documentation for billing and compliance purposes.
