job_description.job_card.job_descriptionTrack missed automatic therapeutic interchange prescriptions and fix and accept T / I accordingly. Review medication profiles to verify the appropriate drug selection, dosage, interactions, and identify potential errors. Recommend therapy changes to physicians and facility staff. Communicate efficiently with nursing homes via phone, fax, and email regarding therapeutic interchange recommendations and high-cost order clarifications. Apply clinical knowledge of medical history, current therapies, and laboratory data to support decision making. Perform additional tasks as assigned by the Cost Containment Supervisor. Demonstrate initiative and interest in expanding knowledge of various pharmacy practices. Licensed Pharmacist in the State of Florida. Long Term Care experience is a plus. Previous experience in Hospital and Retail pharmacy or other medical fields. Strong analytical and critical thinking skills. Excellent written and verbal communication skills. Strong attention to detail. Strong organizational skills. Microsoft Excel knowledge skills. Framework / ECM software knowledge a plus. 9 : 00am - 5 : 00pm, Mondays - Fridays EST
