Responsibilities

In keeping with the Dignity Health core values the Cardiovascular Coordinator will be responsible to coordinate the overall clinical program and ensure smooth operations and patient flow for the clinical activities of cardiovascular services specific to the structural heart program.

  • Provides for the coordination and scheduling of patient care for cardiovascular or cardiothoracic surgery cases.
  • Reviews initial patient referrals to ensure appropriate testing per standing orders set national practice guidelines and

quality standards.

  • Medications dispensed according to all established guidelines.
  • Works with the structural heart team physicians and surgeons makes adjustments to schedule as seen necessary
  • throughout the day and follows up with diagnostic testing and procedures.

  • Performs pre-procedure and discharge teaching history and physicals and follow up phone calls.
  • Review patient record to update medications and treatment plans.
  • Facilitates multidisciplinary conference and ensure records are available to review.
  • Participates in multi-disciplinary team rounding
  • Ensures the coordination of care between the cardiovascular program and other clinical / nursing units within the
  • organization.

    Qualifications

    Education and Experience :

  • 3 years experince in a cardiovascular or critical care role
  • BSN Preferred.
  • Licensure :

  • Registered Nursing License (RN) in state of practice American Heart Association BLS ACLS required.
